Report an Issue

If you would like to report a concern or other issue, please use the anonymous, online form below. You may also visit Internal Audit in person at the District Office, by calling the Department at (714) 438-4602, or you may submit a confidential email directly to InternalAudit@cccd.edu.

You are NOT required to include your name; however, if you would like a status update, leave your name and contact information so that such update may be provided as applicable or permitted by District policy. Failure to provide your name may also hinder any review, evaluation or investigation that may be performed, as access to further information that could be helpful would not be possible.

Whichever means you choose to report the issue, please make sure to specify what type of report it is. There are four categories:

  • Fraud: Any intentional act characterized by deceit, concealment, dishonesty, misappropriation of assets or information, forgery, or violation of trust perpetrated by individuals or organizations to secure unjust or illegal personal or business advantage.
  • Waste: Behavior that is deficient or improper when compared with behavior that a prudent person would consider reasonable and necessary business practice given the facts and circumstances; Intentional or unintentional, thoughtless or careless expenditure, consumption, mismanagement, use, or squandering of Enterprise or District resources; Incurring unnecessary costs because of inefficient or ineffective practices, systems or controls.
  • Abuse: Intentional destruction, diversion, manipulation, misapplication, maltreatment, or misuse of Enterprise or District resources, including the intentional, wrongful or improper use of rank, position or authority that causes the loss or misuse of resources; Extravagant or excessive use as to abuse one's position or authority; Abuse can occur in financial or non-financial setting.
  • Other Wrongdoing: Anything other than the above three. These include but are not limited to:
    • Ethics concerns, potential non-compliance with the District's Ethics policy, and/or other potential conflicts of interest.
    • Events and situations that cause a potential financial loss to the District or inefficient operations.
    • Violation of laws, regulations and/or non-compliance with District policies and procedures.
    • Misuse of District resources.
    • Embezzlement or misappropriation of District assets, employee theft.

 

 


There are some issues related to wrongdoing that are under the purview of Human Resources. If you have a concern about harassment, sexual harassment, hostile work environment, Title IX, or other workplace related issues and employment practices, please report those issues to Human Resources.

The District has authorized Title IX Coordinators at each College with coordinating the District’s responsibilities under Title IX and the related Title IX Procedure (see BP/AP 5910).
